Back on Track is committed to preserving the privacy of all visitors to our website and physiotherapy services. The following privacy policy is provided to inform you on how we use and protect the information that you provide to us either through the website or any other interaction.

All personal information provided will be held in the Back on Track premises at 17 Andover Down Farm, Andover, Hampshire SP11 6LJ.

This privacy policy relates to any and all of the personal data that you provide to by visiting, sending an email via the website or a call back request you consent to the collection of use of the information given.

INFORMATION
WE MAY COLLECT FROM YOU

When you visit, request information or appointments or make enquiries over the phone you may be asked to provide certain information about yourself, including your name, contact details.

For direct debit payments Back on Track Sports Injury Clinic does not hold your card details. We use a third party specialist secure card processing provider.

The internet is not completely secure. We cannot guarantee the security of your personal data transmitted through our website or by email. Any transmission is at your own risk. Some of the information you provide to us may be considered sensitive personal data as defined by the Data Protection Act (“DPA”) 1998. All personal data that you provide to us will only be processed following your explicit consent and always in line with the DPA 1998 and any other applicable data protection legislation.

ACCESS AND UPDATING
PERSONAL INFORMATION

You are entitled to see the information held about you. Records may be viewed on request, please contact Back on Track to arrange an appointment. If a copy is required, there may be a charge for photocopying costs. We will retain your information for as long as you need for us to provide you services. We will retain and use your information as necessary to comply with our legal obligations, resolve disputes, and enforce our agreements.

TESTIMONIALS

We post customer testimonials on our website, which may contain personal information. We do obtain our Clients’ consent to post their name along with their testimonial via verbal consent prior to posting the testimonial on our website. However, if you wish to request the removal of your testimonial, please contact us.

IN CLINIC FORMS

Client information processed includes: Name, address, date of birth, contact number / email address, GP name and address, signature of consent, personal medical insurance (PMI) details. A client does not have to provide their personal details but failure to do so will result in the physiotherapist unable to treat the client as a treatment record has to be maintained for insurance purposes.

The physiotherapist will record the physiotherapy examination onto a paper template form. The client is requested to sign the examination form to provide evidence of consent. If the client requires a treatment plan, this will be emailed to the client. The client will be requested to sign the consent box on the examination form. By providing this information and signing the consent form, the client is deemed to have provided consent for their information to be processed by Back on Track.

The purpose of this information is to allow your therapist to ensure they manage your case with the relevant due care and consideration for your requirements and your plan of care. The PMI details are used to bill your insurer as appropriate or to communicate directly with them if appropriate. Your PMI may ask for reports of your appointments but this will only occur with your consent.

Back on Track processes client data solely for the use of physiotherapy treatment and for no other purpose. Back on Track will not share client information with any outside agency apart from health care professionals to enable the client to progress with their care; the client will provide consent for any referral. You may request us to write a letter to a consultant, GP, school, employer or another person of interest to you. In doing so we will use your name, date of birth and relevant medical details in that letter. You will also receive a copy of that letter should you so choose.

Back on Track will do everything possible to ensure all data is safe. Electronic breaches are not expected as the processed information is paper based. In the unlikely event of fire or theft, diaries and paper records are stored separately to enable clients to be contacted and informed.
